If the facility accepts hazardous waste from an off-site source without a manifest (or the shipping paper allowed for bulk water shipments), and the waste is not excluded from manifesting, the owner or operator must within 15 days of receipt submit an Unmanifested Waste Report (a letter to the Regional Administrator, and from December 1, 2025 an electronic report in the e-Manifest system) giving the facility's EPA ID, name and address; the date received; the generator's and transporter's EPA ID, names and addresses if available; a description and quantity of each unmanifested waste; the method of treatment, storage or disposal for each; the certification signed by the owner, operator or authorised representative; and a brief explanation of why the waste was unmanifested, if known.
