Standard 5: Comprehensive Care – Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition
Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition 5.14-5.20: Care is delivered, monitored and updated against the comprehensive care plan in partnership with patients, carers and families, and end-of-life care follows the National Consensus Statement: patients at the end of life are identified, specialist palliative care advice is accessible, advance care plans are received and recorded, the workforce is supervised and supported, end-of-life care is reviewed against goals, and shared decisions are supported
What does 5.14-5.20 require?
Staff, patients, carers and families must deliver care from the comprehensive plan together, check whether it is achieving the goals, revise it when it is not, and reassess when diagnosis, behaviour, cognition or condition changes. End-of-life care must follow the national consensus statement: patients approaching death identified, specialist palliative advice reachable where such care is given, advance care plans accepted and recorded, staff supervised and supported, the quality of end-of-life care reviewed against the agreed goals, and patients and families helped to make shared decisions.
