Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition 5.07-5.13: Integrated, timely screening and assessment identify risks of harm and social circumstances, Aboriginal and Torres Strait Islander status is routinely asked and recorded, advance care plans are supported, findings and alerts are documented, and shared decision making produces a comprehensive individualised plan with goals, support people, discharge planning from the start, referral and evidence-based care

What does 5.07-5.13 require?

Screening and assessment must be integrated, timely and tuned to the patients and services involved, and must pick up the specific harms the Standard names. Every patient must be asked about Aboriginal and Torres Strait Islander identity and the answer recorded in administrative and clinical systems. Patients must be helped to record advance care plans. Clinicians must screen at presentation, during examination and whenever needed to detect cognitive, behavioural, mental and physical issues and risks and the social circumstances that compound them, assess what screening reveals, record findings and alerts in the record, and through shared decision making produce an individualised plan that matches the seriousness and complexity of the patient's problems, sets agreed goals and actions, names the people the patient wants involved, starts discharge planning from the outset, provides for follow-up referral where needed and reflects best evidence.
