Where people work near holes, penetrations or openings through which a person could fall, a fall prevention device such as a secure fence, edge protection, work platform or cover must be used to provide a safe system of work where reasonably practicable, and openings must be made safe as soon as they are formed, for example with covers, barricades or mesh embedded in the slab (with an extra cover over mesh to stop objects falling through). A cover must be strong enough to stop people or objects falling through and should be fixed securely against dislodgement or removal; penetration covers should carry a point load of at least 2 kilonewtons. Plywood alone is not preferred because it can be mistaken for loose sheets, its fixing is hard to verify and it can be lifted and left unsecured. Covers should be signed, for example DANGER HOLE BENEATH.
