Australia Model Code of Practice: How to Manage Work Health and Safety Risks 2.1b: 2.1 Consult workers on hazards, near misses and unreported incidents

Workers and their representatives should be asked about health and safety concerns and problems they have met in doing their work, and about near misses and incidents that have not been reported. Consultation arrangements should suit all hazard types: workers may hesitate to raise psychosocial matters such as bullying or harassment, so anonymous and confidential channels such as surveys or reporting systems may be needed.
