Procedures are established and maintained for investigating, responding to and acting to minimise the harm from incidents; for looking into and responding to system failures; and for starting and finishing suitable corrective and preventive action. Any change to OHSMS procedures that results from an investigation or from corrective or preventive action is implemented and recorded.
