Continuous improvement and learning programmes include how managers and professionals reason about their own behaviour, not only external systems and structures such as job design, pay, performance review and leadership training. Argyris found that highly educated professionals, rarely having failed, become defensive and project blame when improvement turns to their own performance (the doom loop), and that performance evaluations easily trigger this; teaching people to recognise their reasoning, their espoused and actual theories, and the defences they create is what lets improvement persist.
