A registered provider delivering home support, assistive technology or home modifications (outside specialist programs) must give each individual a written monthly statement by the last day of the following month, even when no services were delivered, showing the subsidy for the quarter or episode, opening and closing balances, supplements, rollover credits, an itemised list of each service or item with name, price, date, units or hours, the individual's contribution and rate, the subsidy claimed and whether an associated provider or third party delivered it, adjustments and refunds, care management hours, committed assistive technology and home modification funds, and home care account and unspent portion balances; it must help the individual understand it, add a usage statement for subscription services, and give a final statement after the last claim. It must also prepare with the individual an itemised individualised budget for an agreed period, with contribution rates, costs and subsidy per service, give it as soon as practicable, and review and reissue it when services, costs or rates change or within 14 days of the individual's request.
