Reports should identify the product, the manufacturer or importer, the nature and extent of the defect, noncompliance or risk and of the injuries or risk, and the reporter, and then provide the fuller information the Commission requests (production and distribution data, incidents, corrective action proposals); written reports should be signed by the chief executive officer unless the responsibility has been delegated in writing to the Commission.
