South Africa Major Hazard Installation Regulations 2022 16: Reporting: the chief inspector informed within 48 hours of a major incident or any incident that activated the emergency plan, a written preliminary investigation report within seven days, a final report within six months, all near misses investigated and recorded in a register open to inspectors and local government, and the supplier of a dangerous substance informed of an emerging major incident or emergency it caused or may have caused

What does 16 require?

Alongside the incident reporting of the General Administrative Regulations (regulation 8 of those regulations), the duty holder informs the chief inspector by telephone, fax or similar means within 48 hours of a major incident or of any incident that triggered the emergency plan, investigates and sends the chief inspector a written preliminary report within seven days of an emergency occurrence or major incident and a final report as soon as it reasonably can and within six months, and investigates every near miss and records it in a register that an inspector and the local government can see at any time. Where a dangerous substance caused, or may have caused, an emerging major incident or emergency occurrence, the duty holder informs the supplier of that substance.
