The organization should regularly review its performance evaluation processes, including the indicators and the actions arising from evaluation, to decide whether they remain suitable, adequate and effective, whether the criteria, results or assumptions they rest on remain valid, and whether negative unintended consequences are occurring, such as under-reporting of incidents to earn a monetary incentive or pressure on workers to avoid punishment or embarrassment. A review can be prompted by internal factors (changes in structure, a merger, a hazard or serious incident that changes operational processes, new processes or ways of working such as remote or flexible working, outputs of worker consultation) or external ones (changes in legal requirements, better monitoring and measurement technology, changed customer expectations), and can take place during regular OH&S meetings, internal audits or management reviews. Issues the review brings up should be acted on.
