Accepting a request for examination shall be treated as an agreement. Each request shall carry enough information to link the patient to the request and the sample without ambiguity, and to identify the requester and how to contact them, the examinations wanted, and the clinical and technical details needed for informed advice. The request can take any format the laboratory and user agree. Where patient care requires it, the laboratory shall contact users to clarify what is being asked for.
