Each entry carries a recommendation (M mandatory; HR highly recommended, with the rationale recorded if not used; R recommended; no recommendation; NR not recommended, with the rationale recorded if used) and a required effectiveness (low, medium or high) to which the measure must be applied if used. Non-mandatory measures may be replaced by others as the table shading allows.
