IATA Operational Safety Audit (IOSA) Standards Manual ORG 3.1: ORG 3.1 Hazard Identification

A hazard identification program implemented and integrated throughout the organization combines reactive and proactive methods; an operational safety reporting system encourages and facilitates reports of hazards, deficiencies and concerns, with analysis, feedback to reporters and action; the Operator should run a confidential reporting system for human-performance events and a program of systematic observation of flight crew performance in normal line operations; and a process identifies internal and external changes that could affect safety risk and manages the risks arising (ORG 3.1.1 to 3.1.5).
