IATA Operational Safety Audit (IOSA) Standards Manual FLT 3.2: FLT 3.2 Flight Crew Responsibilities

The PIC is responsible for recording for each flight the aircraft registration, date, flight number, crew names and duty assignments, departure and arrival points and times, flight time, the nature of the flight, and any incidents or observations relevant to safety (FLT 3.2.1).
