General Practice Standard 2 Comprehensive care – Australia RACGP Standards for General Practices (5th edition)

Australia RACGP Standards for General Practices (5th edition) GP2.4: Transfer of care and the patient-practitioner relationship: the practice transfers care to another practitioner, inside or outside the practice, when the patient asks, and facilitates the transfer when the practitioner asks to end the relationship, in a way that keeps the patient safe

What does GP2.4 require?

Either party may need to end a therapeutic relationship, and the patient must not be left without care. When a patient asks to move to another practitioner within the practice or to another practice, the team facilitates it and transfers the relevant health information with consent. When a practitioner decides to end the relationship, for example because trust has broken down or the patient's behaviour is unacceptable, the practice gives the patient reasonable notice in writing, continues urgent care in the interim, helps the patient find another provider and transfers the record on request, following professional and legal guidance.
