General Practice Standard 2 Comprehensive care – Australia RACGP Standards for General Practices (5th edition)

Australia RACGP Standards for General Practices (5th edition) GP2.3: Engaging with other services: the practice collaborates with other health services to deliver comprehensive care, and its referral letters are legible and contain the information the receiving provider needs

What does GP2.3 require?

No practice meets every need alone. The practice knows the services available to its patients, including specialists, allied health, hospitals, community and mental health services, Aboriginal community controlled services and social supports, and works with them through referral, shared care and case conferencing. Referral letters, whether electronic or printed, are legible and contain the patient's identifiers, the reason for referral and the question asked, relevant history, examination and results, current medicines and allergies, and the referring practitioner's details, in the form the RACGP recommends.
