Australia NDIS Quality and Safeguarding Framework 2016 I-7: Serious incident prevention, internal investigation and reporting

Providers hold primary responsibility for preventing and managing serious incidents: registered providers have internal investigation and reporting arrangements, make sure staff know what a serious incident is and what to do, act immediately to keep people safe, notify police and other authorities where needed, review each incident's impact, preventability and response, and make records available to auditors and investigations; all registered providers report serious incidents (fraud, assault by staff or another participant, culpable neglect, serious unexplained injury, death, unauthorised restrictive practice) to the NDIS complaints function.
