Fitness for duty is reported using the Standard's classifications: Fit for Duty Unconditional, Fit for Duty Subject to Review, Temporarily Unfit for Duty or Permanently Unfit for Duty. If a worker is temporarily or permanently unfit the Authorised Health Professional notifies the rail transport operator immediately by phone to discuss the implications and let the operator make arrangements, discussing only fitness recommendations including any necessary job modifications and not clinical information. In every case the Authorised Health Professional completes the report section of the Request and Report Form without clinical information, giving only the functional assessment of fitness, any recommendation for specialist review or job modification, and any tests the operator must order for future Triggered Health Assessments, such as an audiogram or HbA1c. The Health Questionnaire and the Record for Health Professional are not returned to the operator. For each worker the Authorised Health Professional maintains the completed questionnaire, the completed Record for Health Professional, a copy of the report sent to the operator, copies of relevant supporting information and any additional clinical notes; the records are available to the worker on request, subject to confidentiality, and may be scanned and kept electronically with signatures remaining valid.
