Part 3 Procedures for Authorised Health Professionals: appointments, tests and equipment, orientation, examination, medicines, additional tests, reporting and communication – Australia National Standard for Health Assessment of Rail Safety Workers 2024

Australia National Standard for Health Assessment of Rail Safety Workers 2024 3.2: Tests and equipment: for Pre-placement, change of category and Periodic assessments obtain a resting ECG and non-fasting cholesterol (total and HDL) and HbA1c for Category 1, a urine glucose test at the assessment for Category 2, and audiometry for all categories, with results available at the appointment or a preliminary assessment issued and finalised promptly; use a well-lit, quiet, private and accessible room equipped with a Farnsworth D15 panel, distance acuity chart, audiometer, breathalyser to AS 3547:2019, 12-plate Ishihara test, sphygmomanometer, ECG, drug urine test kit, glycosuria kit and a device for recording data and calculating cardiac risk; the health professional, not support staff, integrates findings into the decision

What does 3.2 require?

For Pre-placement (or change of risk category) and Periodic Health Assessments the required tests are a resting electrocardiograph for Category 1, non-fasting blood tests for total and HDL cholesterol and for HbA1c for Category 1 at Pre-placement and every periodic assessment (feeding the cardiovascular risk calculation for workers aged 30 and over and monitoring existing diabetes), a urine glucose test at the assessment for Category 2, and audiometry for all categories; drug and alcohol testing may also be requested for all categories at Pre-placement or change of category. Results should be available for consideration during the appointment; if not, a preliminary assessment may be issued on the clinical examination and the final assessment made as soon as possible, with the Authorised Health Professional actively pursuing the results and contacting the worker to explain them whether normal or abnormal. Testing for Triggered Health Assessments is determined by the Authorised Health Professional or Chief Medical Officer. Measurements such as visual acuity, audiometry, body mass index and blood pressure may be taken by support personnel, but the clinical assessment and integration of information into a fitness decision belong to the Authorised Health Professional. The examination room should be well lit, quiet, private and accessible, with a Farnsworth D15 panel, a Snellen or equivalent distance acuity chart, an audiometer, a breathalyser complying with AS 3547:2019, Ishihara plates (12 plate edition), a sphygmomanometer, an ECG, a urine drug test kit, a glycosuria test kit and a computer or tablet for recording data and calculating the cardiac risk score.
