Standard 6: Communicating for Safety – Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition

Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition 6.07-6.08: The organisation and clinicians define the minimum information content, service-specific risks and the clinicians involved in clinical handover, and clinicians use structured handover that is prepared and scheduled, has the relevant information, involves the right clinicians, reflects patient goals and preferences, involves patients, carers and families per the patient's wishes, and transfers responsibility and accountability

What does 6.07-6.08 require?

Working with clinicians, the organisation must fix the minimum content of handover from best-practice guidance, the risks particular to the service and to patients and families, and who takes part. Clinicians must hand over in a structured way: prepared and scheduled, with the right information to hand and the right people present, aware of what the patient wants, involving patients and families as the patient wishes, and ending with responsibility and accountability clearly passed on.
