Standard 6: Communicating for Safety – Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition

Australia National Safety and Quality Health Service (NSQHS) Standards Second Edition 6.01-6.04: Governance, quality improvement and consumer partnership systems are applied to clinical communication, and clinical communication processes support effective communication at identification and procedure matching, at transfers of care within and between organisations and at discharge, and when critical information emerges or changes

What does 6.01-6.04 require?

Clinical communication is governed through the clinical governance, improvement and consumer partnership systems, including communication with patients and families in high-risk situations. Processes must support effective communication wherever identity and procedure must be matched, whenever care moves within the organisation, between teams or clinicians, to another organisation or home at discharge, and whenever critical information about a patient, including risk, appears or changes.
