Australia Model Code of Practice: Healthcare and Social Assistance Industry 2.8: 2.8 Notify the regulator of notifiable incidents and preserve the site

Notifiable incidents must be reported to the WHS regulator: a death, a serious injury or illness, or a dangerous incident that exposes someone to serious risk even if it is a near miss. An incident is notifiable only if it arises from the work: an elderly patient dying of natural causes in hospital is not, but a patient dying from a medication mix-up is. When one occurs the PCBU must report it to the regulator immediately after becoming aware and preserve the site until an inspector arrives or directs otherwise, without preventing help for an injured person, removal of a deceased person, making the site safer or assisting a police investigation.
