Where risk is hard to plan for, such as disability support workers in people's homes or a hospital during rapid changes or an emergency, conduct a pre-visit risk assessment for home work, discuss it with the worker and update it as needed. If the environment changes after the initial assessment consider a dynamic risk assessment so the worker can quickly assess and respond. Workers must be trained to do it, and it should be recorded and reviewed in the overall process. Dynamic assessment suits uncertainty and emergencies but does not replace the PCBU's obligations. In the example given, a home nursing provider used a care plan recording known triggers and strategies, paired workers where violence had a history, logged interaction histories and trained workers to assess each visit and end it safely if a threat appeared, then moved future visits to a clinic.
