Australia Model Code of Practice: Healthcare and Social Assistance Industry 2.1a: 2.1 Identify hazards by varied methods and make reporting easy and safe

Use a range of methods and sources to identify hazards: review injury records and workers compensation claims, look for trends, inspect, talk to workers and representatives through meetings or surveys and think through the hazards in different aspects of work. Some hazards are always present, some arise from specific activities and some only when something goes wrong. Provide ways to report hazards, risks, incidents and near misses and encourage use, because absence of reports does not mean absence of hazards. Workers may not report because they see hazards as part of the job, think them minor, lack time, doubt safety is a priority, expect to be ignored, fear blame or disadvantage, do not know how or assume someone else will. Promote a positive reporting culture and address barriers such as casual work, limited English literacy, cultural factors, recent migration and youth, through inductions, translated resources, support, ongoing consultation and a safe environment.
