Safety is not set and forget. Controls must be monitored and regularly reviewed to confirm they work as planned, create no new risks and reflect workplace changes. A review must be done when a control does not work effectively, when more could reasonably be done, before changes that may bring new risks, when new hazards or risks are identified, if consultation shows a review is needed, or if an HSR requests one for those reasons or believes a review was inadequate. Reports, incidents, complaints including informal ones and grievances can reveal new hazards even if not raised as WHS matters, for example concern about excessive hours may signal poor fatigue management. Questions to ask cover whether controls work, risks have changed, workers are consulted and raising concerns and trained, changes are coming, newer controls exist and risks are minimised as far as reasonably possible. If doubtful, go back through the steps with workers.
