Controls must be maintained so they stay fit for purpose, suit the nature and duration of the work, and are installed, set up and used correctly. They should be reviewed regularly and, if not working, must be revised. Under WHS Regulation 38 controls must be reviewed and revised as needed to keep the workplace without risks as far as reasonably practicable, for example: when a control does not adequately control the risk; before a change likely to create a new or different risk the control may not handle; when a relevant or new hazard or risk comes to light; when consultation results show a review is needed; or when a health and safety representative asks for one. Reviews can use workplace inspections, consultation, tests and analysis of data and records. For excavation that is high risk construction work, the SWMS also has to be reviewed and, if needed, revised. Workers and their representatives must be consulted, and any problem found sends the PCBU back through the risk management steps.
