What does 2.4 require?

The operator has arrangements to report, record and respond to incidents and near misses that cause or could cause injury or harm, in proportion to their seriousness, supporting investigation, corrective action and recovery and making sure corrective actions are carried out and watched; the same applies to hazards and non-conformances, and no method, timeframe, record format or reporting system is prescribed. Six things are looked for. Reported or discovered incidents, near misses, hazards and non-conformances get a timely response matched to their seriousness and potential consequences. Where relevant, immediate risks or consequences are dealt with while the deeper causes go to investigation. Investigations work out what happened, how and why, looking for contributing factors and root causes rather than someone to blame. They ask whether systemic issues, work practices, equipment, procedures, human factors or interfaces with other parties played a part. Corrective actions are identified and carried out to stop recurrence here or elsewhere, aimed at underlying causes rather than symptoms. And the findings, outcomes and lessons are passed on so that those affected can apply them in daily work.
