When a Board asks by written notice, a practitioner provides practice information for current and all previous practice: whether self-employed, each premises address and business name and practitioners sharing premises and costs; each entity with which they have a practice arrangement (including volunteer arrangements) with contact details; and any other names used. Failure without reasonable excuse is not an offence but grounds for action.
