Past incidents often followed clear warning signs that never reached anyone able to act. As a core tool, the organization should adopt practices that maximize reporting of conduct or circumstances raising concern about possible violence, which can include: a WVPI policy and complementary policies such as anti-harassment that require and encourage reporting and cooperation with investigations without fear of retaliation; several reporting routes, such as team members, managers above the person concerned, and other nominated HR and security staff, with designated staff trained in handling reports and escalation; training employees to recognize and report concerning behavior; anonymous reporting through a hotline, tip line or electronic means, designed so enough information comes in to allow a meaningful response; a culture that supports good-faith reporting; and qualified staff checking internal incident reports for hints of threat or later violence. What is wanted is information that is correct, full, prompt and honestly given; the faster and more accurately a concern is reported, the better the chance of early intervention.
