Aged Care Quality Standards (Australia) ACQS25-2.12: 15-15(12) Accurate, current and accessible records about the individual

The provider must ensure that information recorded about an individual (a) is accurate and current, and (b) can be accessed and understood by the individual, their supporters, aged care workers, registered health practitioners, allied health professionals, allied health assistants and others involved in their care.
