Each provider and facility must have business processes ensuring timely provision of provider directory information (names, addresses, specialty, telephone numbers and digital contact information of practitioners, and of each medical group, clinic or facility contracted to a network) to plans and issuers, and must submit it at least when it begins a network agreement, when it terminates one, when there are material changes to the information, and at any other time the provider, facility or the Secretary considers appropriate, including on the plan's request. A provider may still require, in its contract or at termination, that the plan remove it from the directory and bear responsibility for inaccurate network status information.
