A plan or issuer that covers obstetrical or gynecological care and requires PCP designation may not require authorization or referral for a female enrollee to obtain such care from a participating professional who specializes in obstetrics or gynecology, must treat that professional's orders for related items and services as PCP authorization, and may only require the professional to follow the plan's procedures (prior authorization for certain services, treatment plans, referral procedures) and to notify the PCP. The plan must include a notice of the PCP, pediatrician and direct-access rights whenever it provides a summary plan description or similar description of benefits (in the individual market, with the policy or certificate); model language is supplied.
