The certification statement must be signed by a person with authority to bind the provider or supplier legally and financially who also has an ownership or control interest: the applying practitioner, the sole proprietor, or for an organisation an authorized official (general partner, board chair, chief financial officer, chief executive officer, president or similar). Initial applications and periodic revalidations must be signed by an authorized official; updates and changes may be signed by a delegated official, provided the delegation was assigned by the authorized official on file, submitted on the enrollment application or electronic process, includes the delegate's title and Social Security number, names an individual with an ownership or control interest or a W-2 managing employee, and is signed by both; individual practitioners and sole proprietors cannot delegate. The signature attests that the information is accurate and that the provider abides by all applicable statutes, regulations and program instructions, and all providers and suppliers are legally responsible for the accuracy, completeness and truthfulness of all information submitted regardless of who completed the application.
