Enrolling: application content, signatures, fees, screening, affiliations and onsite review (424.507 to 424.519) – US Medicare Provider and Supplier Enrollment (42 CFR Part 424 Subpart P)

US Medicare Provider and Supplier Enrollment (42 CFR Part 424 Subpart P) 424.510(a),(d)(1)-(2): Submit a complete, accurate and truthful enrollment application with all required supporting documentation, including licenses, certifications, identifiers and ownership and management information

What does 424.510(a),(d)(1)-(2) require?

A provider or supplier must submit enrollment information on the applicable enrollment application (Form CMS-855A, B, I, O or S, Form CMS-20134, or internet-based PECOS) to the designated Medicare fee-for-service contractor, complete and with supporting documentation, and may be enrolled only after completing the process including any State survey and certification or accreditation. Each application must contain complete, accurate and truthful responses to all requested information for the provider or supplier type; all documentation CMS requires to uniquely identify the applicant, which may include proof of legal business name, practice location, Social Security number, tax identification number, NPI and owners; all applicable Federal and State licenses and certifications (including Federal Aviation Administration certifications for air ambulance suppliers); documentation of the regulatory and statutory eligibility to furnish the items or services; and any other documentation needed to verify the application, including ownership and management. Those enrolling solely to order and certify meet a reduced set of requirements.
