South Africa Occupational Health and Safety Act 85 of 1993 24: Reporting of incidents to an inspector: any incident at work or from plant or machinery causing death, unconsciousness, loss of a limb or part, serious injury or illness likely to be fatal, permanent or disabling for 14 days or more, a major incident, or endangerment by spills, uncontrolled pressure release, machinery fracture or runaway, reported in the prescribed period and manner, and the site of a fatal, likely-fatal or limb-loss incident left undisturbed without an inspector's consent except for rescue and prevention

What does 24 require?

The employer, or whoever uses the plant or machinery, reports to an inspector, in the prescribed time and form, each incident that occurs at work, arises from people's activities at work or is connected with plant or machinery use, in which a person dies, becomes unconscious, loses a limb in whole or part, or is injured or falls ill so that death, a permanent physical defect or an inability to work or carry on their usual activity for at least 14 days is likely; in which a major incident occurred; or in which health or safety was endangered by a spill of a dangerous substance, an uncontrolled release of a substance under pressure, the fracture or failure of machinery producing flying, falling or uncontrolled objects, or machinery running out of control. Where a person died, is likely to die or lost a limb or part, nobody disturbs the site or removes any article or substance involved without an inspector's consent, except as needed to prevent a further incident, remove the injured or dead or rescue people. Road traffic accidents, private household incidents reported to the police and accidents investigated under the Aviation Act are excluded.
