What does 437-003-0404 require?

On construction sites every temporary-power receptacle available to employees that is rated 125 volts single phase at 15, 20 or 30 amperes has approved ground-fault circuit interrupter protection located where the circuit ends at the outlet (listed portable GFCI cords or devices are acceptable); receptacles above that rating have GFCI protection or an assured equipment grounding conductor programme that has a written description, including the employer's own procedures, kept at the job site where the Administrator and affected employees can inspect and copy it, one or more designated competent persons to implement it, a visual inspection before each day's use of every extension cord, device and cord-and-plug equipment for external defects and signs of internal damage (equipment not exposed to damage excepted), and the rule's continuity and attachment tests on all cords, devices, receptacles and grounded cord-connected equipment not part of the permanent wiring, with damaged or defective equipment taken out of use.
