New Zealand Information Security Manual (NZISM) 16.1.42.C.01: 16.1.42.C.01 Logon recording, lockout and account removal rules

Agencies must: record every successful and every failed logon attempt; lock system user accounts once three logon attempts have failed; use a temporary lockout feature to unlock the system up to a maximum number of times that the agency chooses; have a system administrator reset a locked account once that chosen number is exceeded; remove or suspend a system user's account as soon as possible when the person no longer needs access because their role changed or they left the organisation; and remove or suspend accounts that have been inactive for a specified number of days. Agencies may decide the risk of using the temporary lockout feature on their particular systems; the source leaves the number of times as an agency-chosen value shown as [].
