Clause 3.4 Evaluation: performance monitoring, incident investigation, audit, management review – MS 1722:2011 Occupational Safety and Health Management Systems Requirements

MS 1722:2011 Occupational Safety and Health Management Systems Requirements 3.4.2: Incident investigation: documented procedures that find OSHMS failures, notification to authorities where required, competent investigators with employee participation, documented reports, recommendations implemented and verified, and external reports acted on

What does 3.4.2 require?

The organisation must establish, implement and maintain arrangements, including documented procedures, to investigate incidents so that failures in the OSHMS are identified. Work-related incidents must be notified to the relevant authorities where national law requires it. Investigations are carried out by competent persons with appropriate participation by employees and their representatives; the full report is written up and, where that applies, sent to the authorities; recommendations are communicated to the appropriate people for implementation and verification; and reports from external investigative authorities or agencies are acted on in the same way as internal investigations, respecting confidentiality.
