The organisation must develop, establish and periodically review arrangements to monitor, measure and record its OSH performance regularly, assigning who is responsible, accountable and authorised to monitor at each level of management. Indicators are chosen according to the organisation's size, activity and OSH objectives; both qualitative and quantitative measures are considered; and monitoring must be both active and reactive, and recorded. The results must give feedback on OSH performance, show whether the everyday arrangements for identifying, preventing and controlling hazards and risks exist and work, and be used to judge how far policy and objectives are implemented and risks controlled.
