Reviews before a task (pre-job, pre-task or dynamic risk assessment) and after it (post-job debriefs, after-action reviews) can yield a wealth of evaluation information. The organization should consider using them to learn about resource issues (more workers or different skills, equipment repair, whether safety-critical items like ventilation, air quality monitoring and machine guards are available and in good condition), competence gaps and training needs, gaps in fulfilling legal and other requirements, actual working conditions compared with those assumed in planning, how well hazards and risks were identified and opportunities found, how effective the existing controls, procedures and processes are, and whether privacy and personal data are protected in what gets reported. The organization should ensure that the workers doing the task take part in these reviews and should determine the actions needed to keep the information comprehensive and accurate.
