When the organization plans its knowledge management system it shall take account of the issues in 4.1 and the requirements in 4.2 and identify the risks and opportunities it must address so that the system can be relied on to reach its intended outcomes, undesired effects are prevented or reduced and continual improvement is achieved; it shall plan the actions that address them, how those actions are built into and carried out through the system's processes, and how their effectiveness will be evaluated.
