The organisation prepares a plan for monitoring and evaluating how far implementation has progressed against the implementation plan, recommended to look at the actions themselves, what goes into and comes out of them, resources, who holds which role, processes, capacities and anything else relevant. It writes down what monitoring and evaluation found and decides whether the adaptation, and the plan for implementing it, still hold good, updates its policies, strategies and plans from the evaluation, adopts a formal structure for capturing learning to feed current and later policies, strategies and plans, sets how often monitoring and evaluation happen according to those policies and plans, and evaluates monitoring outcomes at suitable stages of the implementation cycle. It also monitors and evaluates ad hoc when relevant new information arrives or a significant event occurs, whether or not major impacts were felt.
