IATA Operational Safety Audit (IOSA) Standards Manual FLT 3.15: FLT 3.15 Flight Crew Reporting Requirements

The PIC reports hazardous flight conditions to ATC without delay, tells the nearest authority, as fast as possible, of any serious incident or accident involving injury, death or substantial damage, notifies the local authority of any emergency action taken in violation of local regulations and submits the required report, and files a special air-report when runway braking action is worse than reported (FLT 3.15.2 to 3.15.5).
