An LQG that has not received the manifest copy signed by the designated facility within 45 days of acceptance by the initial transporter must contact the transporter or facility to find the waste, and if it still has no signed copy within 60 days must file an Exception Report: a legible copy of the manifest and an explanation (a signed cover letter for paper) of its efforts to locate the waste and the results. From December 1, 2025, EPA no longer accepts mailed paper Exception Reports and LQGs must submit them through the e-Manifest system. An SQG with no signed copy within 60 days must submit a legible copy of the manifest with a note that confirmation was not received, through the e-Manifest system from December 1, 2025 (a VSQG shipping as an SQG because of an episodic event still sends it to the Regional Administrator). For rejected loads forwarded by the designated facility to an alternate facility on a new manifest, the clocks run from the forwarding shipment and the alternate facility's signature is what counts. From December 1, 2025, a signed electronic Exception Report kept in the e-Manifest account satisfies retention, and a generator is not liable where only a system technical difficulty prevents producing it.
