A senior executive is given overall responsibility and accountability for data ethics (often the Chief Data Officer); a Code of Data Ethics is created and enacted at that executive's direction; governance structures guide and enforce adherence to the code, such as a chartered data ethics governing body of diverse internal and external stakeholders and a review committee that approves new use cases; and stakeholders are identified and working groups formed to implement it. The code typically commits to doing no harm, interrogating outcomes for bias, using data as its subjects expect, collecting only what a task needs, transparency about collection, use and sharing, design that is explainable and accountable, and continuous ethical review. All personnel, and external stakeholders, have a means to raise concerns, with protection for those who do.
