Australia National Standard for Health Assessment of Rail Safety Workers 2024
Part 2 The health risk management approach: risk categories, assessment types and timing, reporting framework, categorisation steps, authorising health professionals, administration, privacy and quality control – Australia National Standard for Health Assessment of Rail Safety Workers 2024

Australia National Standard for Health Assessment of Rail Safety Workers 2024 2.6.2: Forms and identification: use the model forms, the Request and Report Form (operator to health professional and back, functional findings only), the Worker Notification and Health Questionnaire (operator to worker to health professional, retained by the health professional) and the Record for Health Professional (never returned to the operator), altering only administrative detail and keeping changes consistent with privacy law; ensure the worker can prove identity with photo ID for assessments and pathology; a health professional does not assess without the forms and does not initiate them

What does 2.6.2 require?

The model forms in Part 6 reflect the health management approach and are the basis of the operator's administrative process, on paper or in electronic systems. The Request and Report Form carries the operator's details of the worker, the category and type of assessment and the task-specific requirements to the Authorised Health Professional, who summarises fitness for duty on it in the standard terminology, conveying functional capacity and not medical data, returns it to the operator and normally gives the worker a copy. The Worker Notification and Health Questionnaire tells the worker why the assessment is required and what to do, and contains the questionnaire the worker completes before attending; the health professional reviews it and retains it. The Record for Health Professional guides the clinical examination and, because it holds clinical findings, must not be returned to the operator; the Authorised Health Professional keeps it, and a Chief Medical Officer may request a copy under confidentiality. Administrative detail may be adapted but the report provisions and the Safety Critical Worker questionnaire content are standardised and should not be altered unless additional job demands must be assessed; any change must stay consistent with privacy and health records legislation. The operator also provides the risk categorisation template to the health professional and establishes systems so workers can prove identity with photo ID for assessments and pathology testing.
