Standard 2: Safety and quality systems – Australia National Pathology Standards (First Edition 2027)

Australia National Pathology Standards (First Edition 2027) 2.02: Risk management: establish and operate a risk management system that identifies, reviews and mitigates risks, records safety and quality risks in the risk plan, uses clinical and other data in assessments, acts on risks through the quality and other systems, involves the workforce, reports risks to the governing body, designated person and workforce, evaluates the system's effectiveness and audits it

What does 2.02 require?

Risk is run as a system. The practice identifies, re-examines and reduces risks on a cycle; the safety and quality risks appear in a risk plan; clinical and operational data inform the assessments; mitigation happens through the quality system and the other management systems; staff take part in finding, rating and treating risks; risks are reported upward to the governing body and the designated person and outward to the workforce; the practice checks how well its own risk process works and improves it; and the risk system itself is audited.

Every mapping shown was judged rather than inferred from wording similarity, and the ones that failed review are published too. See the coverage reports and what was rejected.
