A person notifying a systematic review relationship must supply their name and an Australian or New Zealand address, consent to publication of the information, certify that the review followed Schedule 6, and on request give the relevant authority records showing the review was conducted properly and the conclusion is reasonable. A body corporate's certificate must be signed by a senior officer.
