All electrical incidents and electric shocks must be reported immediately to a competent person (regulations may add notification duties), and incidents should be reported even without a shock so procedures can be reviewed. An incident report should record whether it involved death, serious injury or illness, a dangerous event or a near miss; the date and time; the address and specific location; what happened; the person involved, their contact details, occupation and relationship to the reporter; any injury, treatment given and the medical centre or hospital; the PCBU; actions taken or planned to prevent recurrence; and the reporter's name, contact details and position.
